Jefferson County Clerk's Office to reopen Monday, June 29, 2020 at 8:00 am for normal business
The Jefferson County Clerk’s Office is not the local registrar for birth and death records filed after 1966.
If you need a birth or death record filed in Jefferson County between 1903 and 1966, please complete the Application for Birth or Death Certificate form.
Only the person or immediate family can get a birth certificate unless there is a Designation of Representative Authorized to Obtain Vital Record form completed. This authorization must be notarized. Immediate family is defined as mother, father, sister, brother, grandparent, spouse, child or grandchild.
All applications must be accompanied by government issued identification of the applicant. Examples of government issued identification include driver’s license, state ID, passport, etc.
For other birth and death certificates please contact the City Health Departments in the city where the birth or death occurred or contact the Bureau of Vital Statistics in Austin.
