IMPORTANT MESSAGE REGARDING FORMS OF PAYMENT:
Payment of the application fee MUST be in the form of either a
money order, check or cashier’s check made payable to the U.S.
Department of State.
Payment of the processing fee ($35) or photo fee ($12) may be
paid in cash, by credit card or by check payable to the Jefferson
County District Clerk.
